The Freedom of Information and Protection of Privacy Act (FIPPA) applies to Ontario's provincial ministries and agencies, boards and most commissions, as well as community colleges and district health councils. For Ontario hospitals, the FIPPA act takes effect on January 1, 2012.
We are committed to proactive disclosure of information in accordance with the FIPPA act; we will continue to provide additional information and publications to this web section to ensure transparency of our facility and instill confidence in the patients we serve.
Starting on Jan. 1, 2012, you may use the Freedom of Information and Protection of Privacy Act (FIPPA) to access information about your hospital. You will have the right to make a request for access to a range of recorded information held by our hospital, including records of personal information, that came into the custody or under the control of our hospital going back to Jan. 1, 2007. Access to your own personal health records or personal health information, however, will continue to be governed by the Personal Health Information Protection Act.
Please review the Directory of Records to see the types of records held by the Hospital. If the directory does not include the information you are looking for, the information may still be available by submitting a formal request.

You may also make a formal FIPPA request. We will process your request and make information available to you in an expedient and reasonable amount of time, in accordance with FIPPA.
To make a FIPPA request, please complete and submit a request form along with the applicable service fee of $5 (cheques or money orders only please) per request. The service fee should be made payable to the Timmins and District Hospital.
Please note: Additional fees may apply relevant to the cost of research gathering and sending the information requested.
Step 1: Download TADH's FIPPA request form and fill out all the details pertaining to your request.
Step 2: Once you have filled out all the details of your request, print the form.
Step 3: Prepare a cheque or money order for $5 payable to "Timmins and District Hospital".
Step 4: Submit the form and the applicable service fee.
